Patient Information

Appointments

Please call the office during our normal business hours to schedule your appointment. If you need an immediate appointment and you are calling during normal business hours, we will try to accommodate your needs.

Please help us to direct our resources to patients who need our services by calling 24 hours in advance of your appointment to cancel or reschedule. If you fail to show for an appointment more than one time over a six-month period, your patient account will be assessed a $25 charge. If you have another no-show you could receive a dismissal letter from the practice. Please make every effort to make your appointment or call us at least 24 hours in advance to cancel or reschedule your appointment with us.

Triage Nurse

We have a Triage Nurse available during normal business hours to answer questions related to your care. This staff member is designated to assist patients with questions regarding their medications, assess the needs of the patients and coordinate lab and other tests as needed.
